The Remediation and Redevelopment program notifies environmental consultants who use the Remediation Site Operation, Maintenance, Monitoring & Optimization Report (Form 4400-194) that the form’s instructions have been updated.
No changes have been made to the sections for the remedial actions employed.
The “O & M” form is used to document ongoing operation and maintenance of remedial actions at hazardous substance discharge sites under Wis. Admin. Code § NR 724.13 (3). Under the rule, a progress report on operation and maintenance actions is required every six months unless otherwise directed.
While the form continues to have multiple sections for the variety of remedial actions employed, only the applicable sections need to be filled out and submitted to the DNR.
